Lost pin and puk code to new Vodafone you SIM card

The phone is asking me for the pin then it blocked now it wants a PUK code but I don't have it. I have the envelope. The phone number is not on the SIM. Is it on the envelope?

The phone number is always sent on the packaging with the SIM card.
However, to help us, please send us the ICCID (SIM Card Number) and the make and model of the equipment used.

Eu tenho o mesmo problema. Estou na França e o cartão pede o pin mas não o tenho. O que fazer? Preciso urgentemente
